Registration
forms are available for interested parents from each child care site.
There
is a $25.00 non-refundable registration fee per family for the Program.
To
complete the registration form you will need to bring:
1. Parent’s work number, cell number
2. Doctor’s name and phone number
3. Emergency contact names and phone numbers (at least 2 other than
parents)
